BHRS Logbook Submission (2020 & Onwards)

Please complete the below form and upload a copy of your logbook. We will then request sign off from your named assessor(s) before marking and informing you of your results. This should be within 1 month of submission.

Please Note: The logbook changed in 2020.  Please only upload the new version of the logbook here.  If you are submitting the pre-2020 logbook (or an EP logbook) please submit via the BHRS dropbox.

PLEASE ENSURE YOUR NAME AND CANDIDATE ID FORM PART OF THE FILENAME.

  • This can be found by clicking on 'my profile' at the top right of the website.
  • This can be found on your exam registration confirmation.
  • Your assessor(s) must be experienced in device management and ideally hold BHRS certification (previously Heart Rhythm UK certificate of accreditation) or the IBHRE qualification (pacing and devices) or the EHRA CP/AP qualifications. Please only list your assessors once if you only have three don’t fill in the other boxes.
  • Some candidates may have more than one assessor. Please list any additional assessors here. This is optional. Please do not duplicate assessors.
  • Max. file size: 10 MB.
