ICD deactivation in the community

Normally any patients based in the community that required their ICD to be deactivated would need to attend the hospital for this to be done, in normal circumstances we have no provision for attending a patient in the community. We have already had one patient expected to be in the last couple of weeks of life who required his ICD to be deactivated so we have put together a quick guidance sheet.

I cannot see a way to upload it onto here but very happy to share the document if anyone else would find it useful.
